Custom surveys
Write a clinic survey, assign it to a patient, and read their answers without scores or alert bands.
Use Measures to send your clinic's own questions to a patient on iOS or Android. Custom surveys appear alongside standardized check-ins in the assignment picker, but their answers have no total, severity band, or automatic concern alert.
Create a survey
In a consented patient's Measures tab, find Your clinic’s surveys and choose New survey. This is a clinic library, so choose a name your teammates will recognize.
Enter a Survey name, then use Add question. Each question can use a scale, a single choice, or the patient's own words. Choose Create survey to save.
In Send a check-in, open Measure and choose the survey under Your clinic's surveys. Set Repeat to Once, Weekly, Every 2 weeks, or Monthly, then select Send check-in. Creating a survey alone does not assign it.

What the patient sees
The survey opens under its own title with this introduction: your care team put these together. answer what you'd like, skip anything you'd rather not. They can leave questions unanswered, clear a selected scale or choice answer, or close the sheet. send to my care team submits the answers. The confirmation says sent 💛; there is no score or grade.
If sending fails, the form keeps the answers on screen and offers a retry. Do not treat a missing response as a completed survey or as a symptom score.
Read the answers
Completed responses appear under Survey answers in Measures, grouped by survey title with the newest responses first. Each response includes the date, the questions asked, and individual answers. An unanswered question reads skipped. Scale answers show their scale, for example 3 of 5.
Use Edit in the library to change a survey, or its remove control to retire it. Completed responses retain the questions from that administration, so read the wording alongside the answer when comparing responses over time.
Pippa does not total, band, or turn these answers into alerts. Review them directly with the patient. For standardized instruments, see Measures.
